Early Pregnancy Care is a high-yield module. It combines:

  • Acute emergency management
  • Ultrasound interpretation
  • Evidence-based medical treatment
  • Surgical decision-making
  • Ethical, legal and governance principles

This module frequently appears in SBAs, EMQs and clinical communication stations.

Your preparation must therefore be structured and guideline-based.

What the Updated Curriculum Expects

According to the RCOG Core Curriculum, trainees must demonstrate competence in:

  • Diagnosis and management of ectopic pregnancy
  • Management of miscarriage (all types)
  • Recurrent pregnancy loss
  • Ultrasound interpretation in early pregnancy
  • Management of pregnancy of unknown location (PUL)
  • Gestational trophoblastic disease
  • Hyperemesis gravidarum
  • Ovarian hyperstimulation syndrome (OHSS)
  • Communication skills in pregnancy loss
  • Legal & ethical considerations (registration, disposal of remains, safeguarding)

The emphasis is now stronger on:

  • Evidence-based medical management
  • Ultrasound criteria accuracy
  • Safety-netting advice
  • Shared decision-making
  • Outpatient and ambulatory care pathways
How to Organise Your Study

Create one master folder: Early Pregnancy Care

Subfolders:

  • Ectopic Pregnancy & PUL
  • Miscarriage
  • Recurrent Miscarriage
  • Ultrasound in Early Pregnancy
  • GTD, OHSS & Hyperemesis
  • Legal, Ethical & Governance Issues

For each topic:

  • Read the relevant Green-top / NICE guideline
  • Summarise into a single “NOTE” document
  • Add key investigation thresholds
  • Include management algorithms
  • Practise SBAs immediately after completion
  • Second revision → revise your own notes only.

Ectopic Pregnancy (High Priority Topic)

  • You must know about Risk factors; β-hCG discriminatory levels; Ultrasound diagnostic criteria; Definition & management of Pregnancy of Unknown Location (PUL); Criteria for methotrexate; Indications for surgery; Laparoscopic salpingectomy vs salpingotomy; Follow-up protocols

Recent Emphasis

  • Outpatient medical management
  • Clear follow-up pathways
  • Counselling about future fertility
  • Documentation and safety-netting

Miscarriage (Very Frequently Tested)

  • Includes: Threatened; Inevitable; Incomplete; Missed; Septic; Recurrent

Updated emphasis includes:

  • Accurate ultrasound diagnostic criteria
  • Avoiding premature diagnosis of non-viability
  • Shared decision-making in management

Management Options

  • Expectant; Medical (misoprostol ± mifepristone); Surgical (MVA preferred in many settings)
  • For each management options you must know: Indications; Success rates; Complications; Anti-D indications; Infection prophylaxis

Recurrent Miscarriage

  • You must cover Definition: ≥3 first trimester losses (but many units investigate after 2); Evidence-based investigations only; APS screening; Role of progesterone (selected cases); Limited benefit of immunological therapies; Importance of psychological support
  • This area is frequently examined in SBAs.

Ultrasound in Early Pregnancy

  • You must understand Crown-rump length criteria for viability; Mean sac diameter thresholds; Diagnosis of failed pregnancy; Diagnosis of ectopic; Follow-up scan timing
  • Incorrect ultrasound interpretation is a common exam trap.

Final MRCOG Exam Strategy

Early Pregnancy Care can be assessed in both SBAs & EMQs. It can be high scoring if prepared systematically.

  • Know investigation thresholds
  • Memorise management criteria
  • Understand follow-up protocols
  • Apply safety-netting principles
  • Demonstrate empathy in counselling scenarios
