Knowledge Area 10 | Gynaecological Problems

Knowledge Area 10 | Gynaecological Problems

(Aligned with RCOG Core Curriculum 2019)

Gynaecological Problems is one of the most clinically extensive and integrative areas in MRCOG exams preparation. This knowledge area is broad because it reflects real-life practice. There is an overlap of other knowledge areas such as Subfertility, Contraception, Urogynaecology and Gynaecological oncology.

What the RCOG Curriculum Expects

The current curriculum emphasises clinical reasoning, safe decision-making, patient-centred care, multidisciplinary working, and evidence-based management rather than isolated factual recall.

You are expected to demonstrate: Risk assessment and triage; Appropriate investigation strategies; Interpretation of imaging and laboratory tests; Identification of red flags; Safe referral and escalation; Shared decision-making; Awareness of comorbidities (obesity, diabetes, thrombosis risk, mental health); Holistic care

How to Approach This Knowledge Area

Use a Life-Course Framework

  • Structuring revision chronologically improves understanding and recall: Prepubertal; Puberty & adolescence; Reproductive years; Perimenopause; Postmenopause
  • Exam questions are frequently framed within this structure.
How to Organise Your Study

Create a master folder: “Gynaecological Problems”

Subfolders may include:

  • Amenorrhoea
  • Paediatric & Adolescent Gynaecology
  • Abnormal Uterine Bleeding
  • Chronic Pelvic Pain
  • Benign Ovarian Conditions
  • Premenstrual Disorders
  • Menopause & Perimenopause
  • Acute Gynaecology
  • Vulval Conditions
  • Miscellaneous & Emerging Topics

Amenorrhoea

  • A high-yield and conceptually challenging topic.
  • Approach systematically: Hypothalamic causes; Pituitary causes; Ovarian causes; Uterine causes; Outflow tract obstruction; Genetic causes
  • Important areas emphasised in the curriculum: Premature Ovarian Insufficiency (POI) – long-term cardiovascular and bone risks; Functional hypothalamic amenorrhoea; Eating disorders and excessive exercise; Psychological impact and counselling; Limitations of AMH testing
  • Include conditions such as Mayer–Rokitansky–Küster–Hauser syndrome and disorders of sex development (DSD), with attention to MDT input and safeguarding.

Paediatric & Adolescent Gynaecology

  • Increasingly examined and clinically relevant.
  • Key areas: Precocious puberty; Delayed puberty; Menstrual disorders in adolescents; Ovarian cysts in children; Labial adhesions; Safeguarding & child sexual exploitation
  • Be confident with: Consent and Gillick competence; MDT involvement; Referral pathways

Abnormal Uterine Bleeding (AUB)

  • Approach using: FIGO PALM-COEIN classification; NICE Heavy Menstrual Bleeding guidance
  • Core knowledge: Risk stratification for endometrial cancer; Indications for biopsy; Role of outpatient hysteroscopy; LNG-IUS as first-line therapy; Medical vs surgical management; Fibroid pathways; Fertility implications
  • The curriculum highlights: Ambulatory care models; Shared decision-making; Evidence-based surgical selection

Chronic Pelvic Pain

  • Frequently tested in MRCOG exams.
  • Updated emphasis includes: Biopsychosocial model; Central sensitisation; Multidisciplinary management; Rational use of laparoscopy; Avoiding repeated unnecessary surgery
  • Be familiar with pain management pathways.

Premenstrual Syndrome / PMDD

  • You must know Diagnostic criteria; Role of SSRIs; Combined hormonal contraception; GnRH analogues; Indications for surgery

Benign Ovarian Conditions

  • Understand: Ovarian torsion (acute management); Functional cysts; Endometriomas; Surgical decision-making; Impact of cystectomy on ovarian reserve
  • Risk-based and fertility-aware management is essential.

Menopause & Perimenopause

  • Strongly emphasised in the updated curriculum.
  • Core areas: NICE Menopause guideline; Indications and contraindications for HRT; Breast cancer and VTE risk counselling; Unscheduled bleeding on HRT; Genitourinary syndrome of menopause; Bone and cardiovascular health; Management in women with past malignancy
  • Also understand: Premature ovarian insufficiency; Contraception in perimenopause

Acute Gynaecological Problems

  • Often underestimated in revision, so it must not be neglected. You can get EMQs from this topic.
  • It Includes: Acute pelvic pain; Ovarian torsion; Ruptured cyst; PID; Heavy acute bleeding; Sepsis
  • Curriculum focus: Early warning scores; Escalation pathways; Safe discharge criteria

Vulval Conditions

  • Includes: Lichen sclerosus; Chronic vulval itching; VIN (overlaps with oncology); Dermatoses
  • Know biopsy indications and long-term surveillance.

Cross-Cutting & Emerging Areas

  • Increasingly examined themes: Transgender healthcare in gynaecology; Obesity and metabolic health; Ambulatory gynaecology services; Safeguarding and domestic abuse; Ultrasound governance; Surgical innovation and robotics
How to Revise Effectively
  • Create flowcharts for: Amenorrhoea; AUB; Chronic pelvic pain; Acute pelvic pain
  • Prepare “Red Flag” lists for: Postmenopausal bleeding; Rapidly enlarging mass; Severe acute pain; Adolescent safeguarding concerns
  • Integrate major guidelines: NICE; RCOG Green-top; BSGE; British Menopause Society
  • Practise SBAs and EMQs regularly and update notes continuously.
Final Thoughts
  • Knowledge Area 10: Gynaecological Problems is vast, but the examination focuses on: Clinical judgement; Safety; Evidence-based management; Patient-centred counselling; Integration across modules
  • No knowledge area exists in isolation. Questions frequently combine elements of subfertility, oncology, urogynaecology and menopause care.
  • If you organise your notes around clinical pathways rather than isolated topics, this area becomes structured, logical and manageable.

Amenorrhea

•  Very important & tricky topic

•  Divided into primary and secondary amenorrhea

•  Make your own NOTES in the form of FLOWCHARTS so that you do not miss anything.

•  Remember to go in order starting from hypothalamus, pituitary, ovarian, uterine, outflow tract obstruction, genetic, etc.
