Gynaecological Problems is one of the most clinically extensive and integrative areas in MRCOG exams preparation. This knowledge area is broad because it reflects real-life practice. There is an overlap of other knowledge areas such as Subfertility, Contraception, Urogynaecology and Gynaecological oncology.
What the RCOG Curriculum Expects
The current curriculum emphasises clinical reasoning, safe decision-making, patient-centred care, multidisciplinary working, and evidence-based management rather than isolated factual recall.
You are expected to demonstrate: Risk assessment and triage; Appropriate investigation strategies; Interpretation of imaging and laboratory tests; Identification of red flags; Safe referral and escalation; Shared decision-making; Awareness of comorbidities (obesity, diabetes, thrombosis risk, mental health); Holistic care

Abnormal Uterine Bleeding (AUB)
- Approach using: FIGO PALM-COEIN classification; NICE Heavy Menstrual Bleeding guidance
- Core knowledge: Risk stratification for endometrial cancer; Indications for biopsy; Role of outpatient hysteroscopy; LNG-IUS as first-line therapy; Medical vs surgical management; Fibroid pathways; Fertility implications
- The curriculum highlights: Ambulatory care models; Shared decision-making; Evidence-based surgical selection
Chronic Pelvic Pain
- Frequently tested in MRCOG exams.
- Updated emphasis includes: Biopsychosocial model; Central sensitisation; Multidisciplinary management; Rational use of laparoscopy; Avoiding repeated unnecessary surgery
- Be familiar with pain management pathways.
Premenstrual Syndrome / PMDD

Benign Ovarian Conditions
- Understand: Ovarian torsion (acute management); Functional cysts; Endometriomas; Surgical decision-making; Impact of cystectomy on ovarian reserve

Menopause & Perimenopause
- Strongly emphasised in the updated curriculum.

- Often underestimated in revision, so it must not be neglected. You can get EMQs from this topic.

Vulval Conditions
- Includes: Lichen sclerosus; Chronic vulval itching; VIN (overlaps with oncology); Dermatoses
